Commercial Motor Vehicle Drivers
Cornell University, as an operator of commercial motor vehicles is required to be in compliance with the regulations set forth in CFR 49 and NYCRR, Title 17.
Commercial Motor Vehicle Drivers
If you have a vehicle driver with a gross vehicle weight rating of 10,001 pounds or more, the driver is considered a commercial motor vehicle driver and must complete the paperwork below.
The following driver qualification forms need to be sent to the selected applicant, internal or external, with the offer letter:
Driver Qualification
USDOT and NYSDOT require forms for drivers of commercial motor vehicles (CMV).
New Hire CMV Packet which includes the following:
- DQ, Safety History and Drug & Alcohol Checklist (doc) (Drug & Alcohol to be completed for CDL Drivers Only)
- Driver Application for Employment (doc)
- Previous Employer Request (doc)
- Safety Performance History Records Request (pdf)
- Certification of Driver Road Test (doc) (To be completed for Non CDL Drivers Only)
- Certificate of Violations Annual Review (doc)
- Hours of Service Record for First Time or Intermittent Drivers (doc)
Drugs and Alcohol (completed for CDL drivers only)
- DOT Release Form (doc) (internal and external hires)
The following release form must be completed by an employee that has had this requirement added to their job duties.
Vehicle Inspection, Repair and Maintenance
- Driver's Vehicle Inspection Report (pdf) (To be completed by all CMV drivers at the end of their tour of duty driving)
- Vehicle Maintenance File (doc) (To be completed by maintenance staff)
- Inspection Due Date (doc) (To be completed by maintenance staff)
